First our passengers should know how to use the toilets. I have seen a lot of instances when initially the train starts from origin with a clean toilet and after 30 mins someone forgets to flush or puts paper or nappies and blocks the toilet. Also when LHB coaches were first introduced they had good technology like one press flush, tap handles etc. but all those were stolen from the yards and now revert back to the age old chained mugs and hand pressed taps and flushes. Very difficult to fix without public consciousness.

travelled in Panvel Nanded Express recently, the Train's toilet and basin were clean when the rake was shunted on PF6 at Panvel, the mirror was a bit dirty but it was in Okay Condition overall, by the time we reached Pune, the basin was dirty, didn't check toilets as I had to get down at Pune.

IR should start cleaning for every 300 kms or keep OBHS for all trains. Its tough to stop people from using Tobacco products even while traveling. They will make the basin dirty.
